PHOTOGRAPHER'S RELEASE FORM

I _______________________ (Photographer) give Sarah Parker Model & Talent Agency, Inc and its agents permission to use photograph(s) taken by me of __________________________ (Model/Talent) for the specific purpose of promoting and marketing aboved named Model/Talent.

I understand the photograph(s) will be used to promote named Model/Talent in all forms of media including, but not limited to, digital media, portfolios, articles and promotional material for Sarah Parker Model & Talent Agency.

I understand the photographs include e-mailed files and/or actual photographs submitted to Sarah Parker Model & Talent Agency, Inc through the U.S. Postal Service or the Internet.

I give permission for my name to be used in conjunction with the photographs. I have the exclusive right to give permission for the use and publication of these photos.

In giving my consent, I hereby release and hold harmless Sarah Parker Model & Talent Agency, Inc and its agents from any and all responsibility or liability.
